Tesamorelin
Mechanism
A GHRH analog, meaning it doesn't hand your pituitary growth hormone directly. It just tells your pituitary to keep doing its own job, on its own natural pulse. This is a nudge, not a flood.
Key Benefit
FDA-approved to reduce visceral adipose tissue in HIV-lipodystrophy. Also shown to improve cognitive function in mild cognitive impairment (pilot data).
Evidence Base
FDA-approved in 2010, for HIV-associated lipodystrophy. That is the label. Everything you hear about tesamorelin and age-related body composition is off-label use, not what the FDA signed off on.
Route & Rationale
Subcutaneous injection (daily). GHRH analog requires parenteral delivery. Daily dosing matches physiologic GH pulse timing.
Interactions
May alter glucose metabolism. Monitor IGF-1 levels.
Regulatory Status
FDA-approved as Egrifta SV. Filed under GH secretagogue. WADA has it on the banned list, no exceptions.
Hard Contraindications
Cancer (active/history), Pregnancy/planning
